چکیده

Crossover designs in clinical trials are very popular for comparing several non-curative treatments for their eecacy, while having each patient as his/her own control. Many investigators have constructed universally optimal designs for certain repeated measurement experiments. However, most of these statistically optimal designs have certain undesirable features from a clinical perspective. In this work, we compare savings in cost and thus the gain in power attainable from employing crossover designs against completely randomized designs. We note that the assumption of a positive within-subject correlation between measurements has been implicit in earlier comparisons and in most models for repeated measures data. We explore other models for repeated measures data that allow for a negative correlation among repeated measurements. Under such models, we will consider some useful crossover designs and compare their cost eeectiveness with those of completely randomized designs and parallel group designs.

Evidence from crossover trials: empirical evaluation and comparison against parallel arm trials.

BACKGROUND We aimed to evaluate empirically how crossover trial results are analysed in meta-analyses of randomized evidence and whether their results agree with parallel arm studies on the same questions. METHODS We used a systematic sample of Cochrane meta-analyses including crossover trials. We evaluated the methods of analysis for crossover results and compared the concordance of the esti...

Crossover Trials in Diabetes Research

Crossover designs are efficient alternatives to parallel designs. Sample size reduction, comparing the 2x2 crossover to a two-arm parallel study, is approximately 4 to 1 in diabetes clinical trials. One challenge of using a crossover design is understanding and handling potential carryover effects. This paper will review the fundamentals of the crossover design, including design, model, and ana...

Model‐Based Assessment of Alternative Study Designs in Pediatric Trials. Part I: Frequentist Approaches

Alternative designs can increase the feasibility of pediatric trials when compared to classical parallel designs (PaD). In this work we present a model-based approach based on clinical trial simulations for the comparison of PaD with the alternative sequential, crossover, and randomized withdrawal (RWD) designs. Study designs were evaluated in terms of: type I and II errors, sample size per arm...

184-30: Empirical Power for Higher-Order Crossover Designs in Comparative Bioavailability Clinical Trials

A standard 2x2 crossover design has been widely used in drug clinical trials, but it is not optimal because of some undesirable statistical properties such as the low power to test carry-ov er effect. Higher-order crossov er designs address this defficiency and, theref ore, have drawn more attention recently. Howev er, exact power calculation of higher-order crossov er designs poses difficulity...

Efficient crossover designs in the presence of interactions between direct and carry-over treatment effects

Crossover designs, or repeated measurements designs, are used for experiments in which t treatments are applied to each of n experimental units successively over p time periods. Such experiments are widely used in areas such as clinical trials, experimental psychology and agricultural field trials. In addition to the direct effect on the response of the treatment in the period of application, t...
